 Initial Clinic Evaluation Form
This form is ideally to be completed before your first consultation and to be brought along with your referral letter. This will speed up the first section of your time with Mr Achan and thus provide more time during the consultation for specific discussion about your condition, its treatment and the clinical examination.
Specific forms for known conditions. If you know whether your pain is arising from the hip or knee, it may help with reaching a diagnosis if you could fill out the specific hip or knee questionnaires below
Daily Pain Log
If you undergo a diagnostic injection then this log is crucial to your doctor's understanding of the source of your pain and the likely benefits of surgical intervention to the affected joint. Please print this out and keep an accurate record of the level of pain symptoms you experience after the injection. Having recorded this information please remember to bring this along to your follow up consultation with Mr Achan.
